Support Services

Live Well Navigator Service
Part of Swindon Borough Council's offering, Live Well Navigators offer one-to-one support for up to eight sessions, helping you set goals and create a personalised wellbeing plan based on your strengths, interests and needs. Sessions can take place at home or in a community setting that suits you, supporting you to improve your wellbeing and engage more with your community.

Age UK Wiltshire provides a range of services to help older people stay safe, make informed choices and be independent yet connected.

Adult Social Care Team (Swindon Borough Council)
The Adult Social Care Team provides information to older people, adults with physical disabilities and carers, to help make informed decisions and access appropriate services. They can provide help with equipment and adaptations for daily activities, help at home and support for informal, unpaid caring roles.

Support for Carers
If you look after an ill, frail or disabled family member, partner or friend, then you are a carer. Swindon Carers Together can help informal and unpaid carers by providing advice, resources, and emotional support to help carers manage their responsibilities while maintaining their own wellbeing.

Sanford House brings together under one roof, a wealth of help, advice and information for the residents of Swindon. Located on Sanford Street in Swindon, it is a hub for a range of organisations including Citizens’ Advice, the Carers Centre, Healthwatch Swindon and Swindon Mind. They provide information and advice face to face or by telephone.
